From Discrepancy to Alignment: How Quick Collaboration Enhanced Our Reporting Process

When we released our Timely Follow-up Report to a care team at a value-based care (VBC) organization, we were optimistic it would support their oversight efforts.

The report was designed to monitor the care team’s performance in ensuring timely follow-ups for every discharged patient, tracking all outreach efforts (e.g., calls, messages) and outcomes (e.g., successful follow-ups).

Soon after, the team conducted a detailed review, comparing our report with their EHR data, and flagged several discrepancies in patient assignment records.

We immediately re-validated the data on our side, which appeared accurate. To move quickly and align perspectives, we pulled together a 30-minute cross-functional meeting including the VP, team leads, and our analysts. During the call, the group walked through specific examples and compared backend logic. The VP, who had oversight across both teams, promptly brought in additional team members to help clarify workflows. Together, we identified that a key step had been skipped by some team members in one team, leading to the inconsistencies.

By the end of the session, we had a clear action plan in place to align the workflow and update internal practices. This change enabled the care team to better manage timely follow-ups and reduce readmission risks.
